Youth and Children's Ministry Injury Report Form

Please fill out this form any time a child, youth, or volunteer sustains an injury during Children's or Youth Ministry programming. For our purposes, we'll define an injury as anything requiring more than a band-aid and a hug, especially an injury that requires parents to be summoned and/or immediate or subsequent medical care. Please fill out this form on the same day as the incident, when possible.
